Filling the CDPAP form to check the eligibility for CDPAP

Our CDPAP form contains three portions:

• Patient Information
• Patient Insurance Information
• Caregiver/Aid Information

Here is a step-by-step guide for you to fill out the CDPAP form.

In the first column of patient information of the CDPAP form, you need to add your first and last name separately on the said box. Choose your gender (male/female). Add your phone number and email address along with your residential address. In the address column write your street address and in the second box write down your apartment or suit. Write your city name and in the second box write your state or province. In the last box of patient information write the postal or zip code of your area. Search on the internet if you don’t know your postal or zip code.

In the Patient’s Insurance Information, fill the box with the Medicaid Client’s SSN or Id Number. Choose the birth date of the patient and write in detail the health situation a patient is feeling. This is all you need while filling in the Insurance information of a patient.
